LOUIS SCHMITZ FOUNDATION STUDENT SCHOLARSHIP APPLICATION

Louie Schmitz was a local Farmington man that started youth hockey and coached and supported many other youth sports in Farmington. His involvement in youth sports and the community was a platform to support children and to change lives. He cared about kids and felt that they should be involved and busy, whether playing sports together with other children or taking part in community activities. He believed in commitment and fair play and that all children should have an opportunity to grow and learn and to be part of a team. He unselfishly devoted his time for many years and was an advocate for Farmington youth. The Louis Schmitz Foundation was created to uphold his memory and to continue the tradition of supporting Farmington.

The Louis Schmitz Memorial Scholarship is one of the ways that The Louis Schmitz Foundation gives back to the community. The memorial scholarship is presented each year to one male and one female graduating high school senior that live within the Farmington Area Public Schools District 192 borders.

APPLICATION DEADLINE  – MAY 8, 2024, 8:30 AM

The Louis Schmitz Foundation Scholarship Committee (LSFSC) will determine your eligibility and merit to receive financial assistance based upon the information you provide in your completed scholarship application. All sections are required to be completed with the exception of the Extenuating Circumstances Section. It is critical that the information you provide is accurate and complete. The application must be completely filled in order to be considered for review by the committee.

Fill out the on-line application below or download the Louis Schmitz Memorial Scholarship Application 2024 and return by the due date to the Farmington High School Guidance Office in order to be considered for review by the committee.

    Please list school activities - including sports, clubs, activities and any leadership roles. Please include any awards, letters or honors you have received during your high school career (grades 9 through 12).

    Please list any activities that you were involved with outside of school. These would include: volunteer, work experience, any non-school sports (club or athletic association), church or other organizations such as Boy or Girl Scouts, 4H, etc. Please include number of years of participation and special recognition or leadership positions held. Also include your supervisor’s name for work experience or organization where you volunteered.
  • Career Plans

    Please describe your career aspirations and goals and how you would plan to attain them. Example: I would like to become a school teacher and plan to go to college and earn a teaching certificate. During my time in college I would plan on having summer jobs to help pay for my college and consider tutoring students to gain experience. Page other organizations such as Boy or Girl Scouts, 4H, etc. Please include number of years of participation and special recognition or leadership positions held. Also include your supervisor’s name for work experience or organization where you volunteered.
  • Extenuating Circumstances - Optional

    If there are extenuating circumstances you would like to share with the selection committee, please include them in this section. Examples would be financial constraints, medical expenses, additional family members pursing post secondary education of any type, etc.

    By checking the boxes below, the Parent/Guardian allows for release of student applicant’s transcript which will be copied and submitted by the Farmington High School Guidance Office.
